Our physician-directed CKD stage 4 workshop series “My Kidneys. My Options. My Life.TM” has been shown to improve patient outcomes by empowering patients to make informed decisions about their health. Packaged in an easy-to-deliver format, the program is also easy to implement and offers physicians all the tools they need to increase home dialysis, decrease dialysis catheters, to give patients hope, improve patient health and quality of life, increase patient satisfaction with the potential to increase practice revenue.

Benefits & Outcomes

With over 6,000 workshops conducted to date, Raenali Publications® innovative, CKD stage 4 patient education program has been shown to deliver consistent results for patients and physicians.

INCREASE HOME DIALYSIS

The percentage of patients opting for home dialysis (both home hemodialysis and peritoneal dialysis) increased significantly after attending three or more workshops.

CKD4 Workshop Program Details

Each workshop includes a binder with take home materials, a poignant 15-20 minute video and a one-on-one session with a physician educator, who reviews the topic, answers questions and helps the patient create an actionable plan to address each topic. The physician educator is also trained to help the patient set goals and decide on the best and most appropriate treatment plan to meet those goals. The program also facilitates an open dialogue about options for renal replacement therapy, including transplantation and home dialysis.

WORKSHOP 1

Learning About My Kidneys and Kidney Disease

Patients will learn: Normal kidney function, chronic kidney disease, its causes and diagnoses, stages of CKD, treatment goals to prevent CKD progression and how to manage complications, symptoms of CKD, plus a brief description of RRT

WORKSHOP 2

Learning What I Can Do About Kidney Disease

Patients will learn: Importance of patient involvement, six complications of CKD, detailed management of the complications with an action plan

WORKSHOP 3

Learning About My Options - Transplantation

Patients will learn: All about the transplant process, its advantages and disadvantages, the transplant evaluation, transplant surgery and post transplant care, as well as how to decide whether transplant is a good treatment choice

WORKSHOP 4

Learning About My Options - Dialysis

Patients will learn: All about Dialysis, Types of Dialysis, how each works and pros/cons-in-center hemodialysis, home hemodialysis (short daily and nocturnal), peritoneal dialysis. Types of access, how to choose a dialysis modality, travel/work, finances

WORKSHOP 5

Learning About My Options - Informed Decisions and Conservative Treatment

Patients will learn: Their right to choose the best treatment option and the option to have conservative treatment, what a trial period on dialysis is, why it is important to have an Advance Directive and their types, support services including hospice

WORKSHOP 6

Living My Life - Learning to Live Well with CKD

Patients will learn: Connection between emotions and physical health, outline of common emotions (sadness, anxiety, anger, isolation) and coping skills to deal with each emotion, answers to the most common questions about CKD and the importance of an action plan

PATIENT SURVEY RESULTS

The vast majority of patients were very satisfied with the workshops because they received clear, reliable information in a comfortable, physician-directed environment. Higher patient satisfaction can also help improve a practice’s overall satisfaction scores.

The catheter rate dropped from 68% for patients who did not attend any CKD stage 4 workshops to 60% for patients who attended one or two workshops to 44% for those who attended three or more sessions.

Rates of in-hospital dialysis initiation dropped from 85% for patients who attended no CKD stage 4 workshops to 78% for those who attended one to two sessions to 65% for patients who attended three workshops or more.

The incidence of patients opting for home dialysis (both home hemodialysis and peritoneal dialysis) continued to increase as patients learned more about their disease—from 8% without education to 13% following one to two CKD stage 4 workshops to 19% after three or more sessions.

On average, practices that offered the CKD stage 4 workshop program in which the nursing staff took an active role reported a decrease in patient no-show rates from 32% to 12% for patients who participated in the program.
