“My Kidneys. My Options. My Life.™” Workshop Program
“My Kidneys. My Options. My Life.TM” is the only chronic kidney disease patient stage 4 educational program that utilizes different learning and teaching methods. This educational workshop series—which was developed in conformance with current CMS guidelines for reimbursable CKD education—includes six modules on a variety of topics that are taught one-on-one at the nephrologist’s medical practice.


CKD4 WORKSHOP PROGRAM INCLUDES
CKD4 Workshop Program Details
Each workshop includes a binder with take home materials, a poignant 15-20 minute video and a one-on-one session with a physician educator, who reviews the topic, answers questions and helps the patient create an actionable plan to address each topic. The physician educator is also trained to help the patient set goals and decide on the best and most appropriate treatment plan to meet those goals. The program also facilitates an open dialogue about options for renal replacement therapy, including transplantation and home dialysis.

How do CKD Stage 4 patients benefit?
Empowerment
Once patients better understand their chronic kidney disease, they can make informed decisions about lifestyle choices and treatment. Through education, patients learn why they must take an active role in managing their CKD—day-to-day and for the long-term.
They also feel empowered. The most frequent comment patients share in the follow-up surveys is: “I’m so glad to have this information and am ready to make decisions and changes.”
IMPROVED HEALTH OUTCOMES
Patients also learn to make better choices regarding their health. This leads to improved health outcomes, including:
- Increase in home therapy, i.e. home hemodialysis and peritoneal dialysis.
- Fewer catheters at dialysis initiation.
